This year we would appreciate it, if you would submit one form for each person attending. This will benefit both the organizers and the attendees, because we will be able to have more name tags prepared in advance.

Also, if you plan on entering your greyhound in the Fun Run, please make sure your correct mailing address is on this registration form. This is the address we use to mail the DVDs.

Children 12 and under are FREE to all events
